For a truly personalised experience, we assess your skin at cellular level and tailor your facial using a combination of skin healing treatments to target your key concerns. We choose from highly bespoke peels, micro-needling, hydration masks, LED light therapy, peptides and plant stem cells to help you achieve a healthier, brighter complexion.
TERMS & CONDITIONS
1. HUM2N Skin Lab Membership
1. HUM2N Skin Lab Membership offers the following treatment for the duration of at least 3 months at £195 per month.
> Monthly Prescription Facials (as recommended by your Senior Aesthetician)
> Monthly Consultations
> 10% off medical-grade skincare
2. Membership Payment Schedule
1. HUM2N Skin Lab Membership are required to pay £395 per month until the end of the 3 months membership term. Once paid, the Membership Fee is non-refundable, except as set forth in the Membership Cancellation Policy.
Upon approval of your membership, your membership fees become payable immediately and will recur on a monthly basis until the end of your membership term with us. By providing us with your payment details, you accept and consent to being charged with membership fees in the form requested by HUM2N. Once you have paid your first instalment of your 1. HUM2N Skin Lab Membership, you must book your first appointment within 30 days.
Membership continues on a rolling basis until you notify us with at least thirty (30) days' notice via written confirmation on email email@example.com.
Membership Options may change from time to time, and you will receive at least thirty (30) days’ advance notice of such changes. However, you are entitled to the full scope of your Membership Option as it existed as of the effective date of a specific Membership Term for the duration of such Membership Term. For any subsequent Renewal Term, you may accept the revised Membership Options or reject such and terminate your Membership.
3. No Emergency Care; Certain Services and Items Excluded.
If you have an emergency you must dial 999. HUM2N does not treat emergencies. HUM2N does not offer specialist medical services or medications under our Health Programmes.
4. No Insurance Accepted; Self-Payment Only.
The 1. HUM2N Skin Lab Membership is a direct health optimisation care service; it is not health insurance. HUM2N does not participate with or bill commercial health insurance plans. HUM2N may recommend you receive services not offered by our company (e.g. specialty services, diagnostic tests), but in no event will HUM2N be responsible for any resulting medical bills.
You are solely responsible for payment of all fees for HUM2N’s services. If you do have health insurance, your insurance policy is a contract between you and your insurance company. It is your responsibility to know your benefits, and how they will apply to your benefit payments. HUM2N takes no responsibility to understand or be bound by the terms and conditions of such insurance. There is no guarantee your insurance company will make any payment on the cost of the services you have purchased.
5. Membership Billing.
In order to participate in the 1. HUM2N Skin Lab Membership, your Membership Fee payments will be charged to your credit card on a recurring basis. You hereby agree to allow HUM2N to securely store your credit / debit card information (the “Payment Method”).
You authorize the Payment Method to be used automatically for your payment responsibilities to HUM2N. If a credit card account is being used for a transaction, HUM2N may obtain pre-approval for an amount up to the amount of the payment. If you want to designate a different payment method or if there is a change in your Payment Method information, you can change the information with HUM2N. This may temporarily delay your ability to make online payments while HUM2N verifies the new payment information. You represent and warrant that: (1) any credit / debit card information you supply is true, correct and complete, (2) charges you incur will be honored by your credit/debit card company, (3) you will pay the charges incurred in the amounts posted, including any applicable taxes, and (4) you are the person in whose name the credit / debit card was issued and are authorised to make a purchase or other transaction with the relevant credit / debit card and information. You agree and authorise the Payment Method to be billed automatically in accordance with the Membership Fee Payment Schedule in an amount equal to the Membership Fee in effect for your Membership Term.
If HUM2N is unable to secure funds from your debit / credit card(s) for any reason, including, but not limited to, insufficient funds in the debit / credit card or insufficient or inaccurate information provided by you when submitting electronic payment, HUM2N may undertake further collection action, including application of fees to the extent permitted by law.
You have the right to revoke this authorization by contacting HUM2N at firstname.lastname@example.org at least fifteen (15) days prior to the scheduled payment date. You understand that your Membership may be cancelled or withheld if you revoke this authorization, and you are still responsible for all charges you incur or otherwise owe to HUM2N. This authorization will remain in full force and effect until revoked by you or HUM2N.
6. Membership Term
HUM2N may, in its sole discretion, not accept this Agreement and return your payment to you. If HUM2N accepts the Agreement, it will notify you, and the initial term of this Agreement will begin on the date HUM2N receives your Membership Fee payment and will last for 6 months, and thereafter is a rolling monthly agreement with no fixed termination date.
7. Cancellation policy
You are able to cancel the membership within the first 14 days aka ‘the cooling-off period’ , and the amount of services taken in relation to the membership, will be deducted from initial payment received, or a pro rata amount will be refunded. If you want to cancel your membership within the cooling-off period, you must send written notice to email@example.com; with a valid explanation.
After the cooling-off period you may cancel your membership within your membership commitment period for the reasons set out in – 7.1 to 7.5 only. We may require you to produce proof, satisfactory to us, if you cancel for any of the following reasons:
– 7.1 Pregnancy
– 7.2 Serious illness
– 7.3 Serious injury
– 7.4 Redundancy
– 7.5 If we significantly reduce the opening hours or services at HUM2N, unless this is temporary, and we need to do so for (i) health and safety reasons (ii) for maintenance or (iii) for improvements that will benefit a majority of members – in any such case we will offer other facilities within your centre or at another centre.
8. Electronic Communications.
By providing your email address, you agree to receive electronic communications via email. You can choose to opt out by emailing firstname.lastname@example.org
9. Privacy and Confidentiality.
HUM2N and its providers will maintain a record of the services they provide you, and will maintain the confidentiality of your medical information in accordance with governing law.
10. Entire Agreement; Amendment.
This Agreement sets forth the entire agreement between the parties with regard to the subject matter hereof, and supersedes all prior or contemporaneous oral or written agreements. This Agreement may be amended only in writing signed by all parties. Notwithstanding the foregoing, HUM2N may, upon at least sixty (60) days’ notice to you, unilaterally amend the Membership Fees and Membership Payment Schedule at any renewal period of this Agreement and/or amend this Agreement if required by applicable law. Upon receipt of such notice, you may accept these changes or reject them by emailing us email@example.com
11. Appointment No Show / Cancellation / Rescheduling Policy
If you wish to reschedule your appointment, you must reschedule over 24 hours in advance of your scheduled appointment. If you cancel within 24 hours of your appointment, we will deduct your treatments from your account. If you do not show up for your scheduled appointment without informing us, we will deduct this treatment from your account.
12. Exchange Policy
Your membership includes 1 skin consultation and 1 prescription facial as recommended by your Senior Aesthetician per month only for the duration of 6 months and cannot be exchanged for other products and services under this membership.
13. Miscellaneous. Governing Law.
In the event of any dispute between you and us concerning these Terms, the laws of England will apply. If you wish to take court proceedings against us, you should do so within England, except to the extent that applicable law in your country of residence requires mandatory application of another law and/or jurisdiction and/or language, and such requirement cannot lawfully be excluded under these Terms, in which case such law and/or jurisdiction and/or language shall apply, as far as legally required.
